Only an education format to allow individuals to make informed decisions of their desire should an end of life scenario or emergency arise.
Q: What is advanced care planning?
A:
Advanced care planning is making choices and decisions about specific care an individual may want in the event one became incoherent, unable to speak on their own behalf or are in the end of life stage. Advanced care plans are choices an individual makes for themselves based on their own personal choices, beliefs and values.

Medicare may also cover this service as part of your medical treatment. When Advanced Care Planning isn’t part of your yearly “Wellness” visit. The part B deductable and coinsurance may apply.
For individuals not on Medicare B. Advanced care planning cost are subject to location of residence and income.
